SSA FOR SCHOOL-AGE CHILDREN

RESPONSIBILITIES/JOB DUTIES:
Supports eligible children (grades K through 6th grade).
SSAs attend School IEP & multi agency team meetings, connect families to available resources and collaborate with child-serving community providers to address the needs of the child and his or her family.
Collaborates with children, their families, and IEP team to identify abilities, interests , needed supports, resources, and training during their school years.

Collaborates with community agencies including Children Services, Family and Children First Council, Children's Hospitals/ medical providers, Counselors, Interagency Review Council , Respite, Residential, Mental Health Social Security, Medicaid, and Family Support Services to assure they have access to community and resources Coordinates supports identified for individuals transitioning from an institution into the community Engages with families to continually assess risks and needs Assists with guardianship issues, with duties that can include assisting with the application process, attending hearings.
Provides follow-up discussions via phone, email, video, in-person or other contact in order to ensure health and welfare to help the person live their best life; Collaborates with child-serving agencies in the community to coordinate services that help facilitate community integration; Provides support to families with educational issues, and attends IEP or 504 meetings to support families; Utilizing Person-Centered Planning, coordinates services and supports for community integration through various funding sources; Provides continuous monitoring of authorized services and reviews the children's progress; Assures that assigned assessments identified by County and State rule requirements are completed and/or coordinated, as applicable; Completes Targeted Case Management (TCM) notes as needed.
Adheres to work assignment timelines Provides excellent customer service in order to support the Board's mission and vision.
Coordinates housing for individuals transitioning from an institution into the community; Follows all State and Federal rules as well as Board policies and procedures; Provides crisis assistance to ensure that needed services occur in a critical or unexpected situation (e.
g.
:
court appearances, arranging for respite, addressing behavioral or medical emergencies, on-call responsibilities, etc.
); Reports major unusual incidents to include alleged abuse/neglect in accordance with HB 403, Ohio Revised Code 5123, Administrative Rule 5123:
2-17-02, and County Board policies and procedures; reports alleged abuse/neglect to Children Services.
Assists the individual with due process when necessary; Adheres to a flexible work schedule designed to meet individual needs; PRE-EMPLOYMENT REQUIREMENTS:
Bachelor's Degree Or Graduate-Level Degree From An Accredited College or University 1 Year Of Experience In Case Management Or Equivalent Experience Required Valid Driver's License With Fewer Than 6 Points Must Be Flexible/Adaptive/Supportive To Meet The Needs Of Individuals/Providers Must Be Accessible During Declared Work Schedule, And For On-Call Rotation Experience With Computer Programs Including Database Programs, Word Processing, Spreadsheets And Other Computer Software Programs Preferred Must Be Able To Lift And Carry Up To 20 Lbs.
